Job summary

21st Century Home Health Services Inc. in California is seeking a Part Time Physical Therapist Case Manager to deliver in-home therapy across Contra Costa County.

You will assess patients in their homes, develop care plans, and coordinate with other clinicians to support safe, independent living. Responsibilities include home evaluations, mobility restoration, detailed treatment plans, physician communication, and progress reporting.

Qualifications

• Current CA Physical Therapist license
• BLS/CPR certification required
• Valid CA driver’s license
• Knowledge of local community resources required
• Home health care experience preferred

Responsibilities

• Perform home evaluations and develop a care plan with PTA supervision
• Help patients regain mobility and independence
• Create detailed treatment and care plans for each patient
• Communicate and coordinate with patients’ physicians to ensure quality care
• Evaluate and report patients’ progress to the MD
