Job summary

21st Century Home Health Services Inc. in California seeks a Part Time Physical Therapist Case Manager to evaluate and address the needs of individuals in their homes, helping them live safely and productively at home.

You will work with clinicians to restore function, develop care plans, and coordinate with physicians to ensure high-quality care while maintaining a patient-centered approach.

Qualifications

• Current CA Physical Therapist License is required.
• BLS/CPR certification required.
• Valid CA Driver's License required.
• Knowledge of local community resources is helpful.
• Experience with home health case management is preferred.

Responsibilities

• Perform home evaluations and develop care plans with PTAs.
• Help patients regain mobility and independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate and report progress to MD.
