Job summary

21st Century Home Health Services is hiring a Part Time Physical Therapist Case Manager for Contra Costa County (Richmond area). You will evaluate patients in their homes, develop care plans with PTAs, and coordinate with physicians to restore function and independence.

The role requires a CA PT license, BLS/CPR, and a CA driver’s license. You’ll work in a supportive, patient‑centered team with flexible schedules and robust benefits.

Qualifications

• Current unrestricted CA Physical Therapist License required.
• BLS/CPR Certification required.
• Valid CA Driver's License required.
• Knowledge of local community resources helpful.
• Experience with interdisciplinary teams is highly desirable.
• Experience in home health care preferred.

Responsibilities

• Perform home evaluations and develop care plans with PTAs.
• Help patients regain mobility and independence in the home.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality care.
• Evaluate and report patient progress to MD.
