Job summary

21st Century Home Health Services is seeking a Part Time Physical Therapist Case Manager to evaluate and address the needs of individuals in their homes across Contra Costa County, including Richmond. You will work with an interdisciplinary team to restore function and promote independence in the home setting.

Qualified candidates have a CA PT license, BLS/CPR, a CA driver's license, and knowledge of local resources.

Qualifications

• Must hold CA Physical Therapist license and BLS/CPR.
• Valid CA Driver's License.
• Experience in home health care is preferred.
• Knowledge of local community resources is required.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Perform an evaluation in the home and develop a care plan with PTAs.
• Help patients with mobility, movement, and regain independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
