Job summary
21st Century Home Health Services (21HHS) in Santa Rosa, CA is seeking a Part Time Physical Therapist Case Manager to evaluate and treat patients in their homes across Contra Costa County. You will collaborate with clinicians, develop care plans, and help patients regain mobility.
The role requires a CA Physical Therapist license, BLS/CPR certification, a valid CA driver’s license, and strong communication skills; flexible schedules and field-based work with company devices and CEU benefits.
Qualifications
- Current unrestricted CA Physical Therapist License is required.
- Current BLS/CPR Certification is required.
- Valid CA Driver's License is required.
- Knowledge of local community resources is required.
- Experience in home health care is highly preferred.
Responsibilities
- Perform home evaluations and develop care plans in partnership with PTAs.
- Help patients improve mobility and regain independence.
- Create detailed treatment plans for each patient.
- Coordinate with patients’ physicians to ensure quality care.
- Document patient progress and communicate with the care team.
- Demonstrate empathy and strong interpersonal skills in field visits.
