Job summary

21st Century Home Health Services (21HHS) is seeking a Part Time Physical Therapist Case Manager to evaluate and address the needs of patients in their homes in Contra Costa County, including Santa Rosa, CA. You will work with clinicians to restore function and promote safety in the home environment.

You will evaluate and treat patients with medically prescribed PT programs, supervise PTAs per agency standards, and coordinate care with physicians.

Qualifications

• CA PT license required and in good standing.
• BLS/CPR certification required.
• Home health care experience preferred.
• Strong communication and collaboration with interdisciplinary team.
• Self-directed with empathy and critical thinking.

Responsibilities

• Evaluate patients in their homes and develop a care plan with PTAs.
• Help patients improve mobility and regain independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to the physician.
