Job summary

Haven Home Care is expanding its Eastern Ohio footprint and seeks an LPN for supervisory visits across Ashtabula, Columbiana, Mahoning, and Trumbull counties. You will conduct supervisory visits, process documentation, and administer pre-employment testing for aides.

This part-time expansion role emphasizes building client base with flexible scheduling and mileage reimbursement as you establish services in a new territory.

Qualifications

• Must be a licensed LPN in Ohio and in good standing.
• Graduation from an accredited School of Practical Nursing.
• Current CPR certification; valid driver's license and reliable transportation.
• Residence in Trumbull or Mahoning County preferred.
• One or more years of home health agency or hospital experience; scheduling/docs/compliance a plus.

Responsibilities

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.
