Job summary

Haven Home Care Inc. is expanding in Eastern Ohio and seeks an LPN to handle open and supervisory visits across Ashtabula, Columbiana, Mahoning and Trumbull counties. This is a per-visit role with pay per supervisory visit, open visit, skills test, and office time, plus mileage reimbursement. Initial hours are modest as the client base grows.

The candidate should be currently employed, flexible, entrepreneurial, and able to travel and document visits accurately. No guaranteed weekly hours.

Qualifications

• Must be at least 18 years old and graduate from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing and CPR certification.
• Valid driver's license, reliable transportation, and auto insurance for work.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and supervise Home Health Aides every 60 days.
• Prepare accurate documentation and review regulatory requirements.
• Conduct pre-employment skills tests and written nursing tests.
