Signify Health, Inc. is seeking a Part Time Physician to provide In-Home Health Evaluations, visiting patients in their own homes and guiding care with a 45-minute visit that includes a medication and medical history review, a physical exam, and optional diagnostic testing as ordered by the health plan.

You'll use an iPad with a clinical workflow and may conduct virtual visits when needed. Active, unrestricted MD/DO licenses in the coverage area are required, with travel across licensed states

Qualifications

• Active, unrestricted license in the coverage area(s).
• MDs or DOs eligible for the role.
• Experience with in-home health evaluations preferred.

Responsibilities

• Conduct in-home health evaluations (~45 minutes) including medications and medical history review.
• Perform physical evaluation and document findings.
• Coordinate with health plans to order additional diagnostic tests when appropriate.
• Utilize iPad and clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
