Job summary

Signify Health Medical Associates, PLLC seeks a Part Time Physician to provide In-Home Health Evaluations, engaging with patients in their homes and bridging gaps in care. You’ll use an iPad and a clinical workflow to review medical history, perform a physical evaluation, and coordinate with health plans.

Anticipated weekly hours around 29.5 with potential local travel across licensed states, plus occasional virtual visits.

Qualifications

• Active, unrestricted MD/DO license in coverage area (Yakima).
• Multi-state licenses (preferred).
• MDs and DOs eligible for the role.

Responsibilities

• Conduct in-home visits with patients and review health history.
• Perform physical evaluations and discuss care plans with patients.
• Coordinate with health plans and Signify Health clinicians.
• Travel locally as needed; some virtual visits may be required.
