Job summary

Signify Health, Inc. is seeking a part-time physician to perform in-home health evaluations for health plan members in their homes across licensed areas. You’ll engage with individuals, review medications and medical histories, perform concise physical evaluations, and facilitate connections to appropriate care.

The role involves 29.5 hours per week, travel within licensed states, and the use of an iPad-based workflow. Prior MD/DO licensure and willingness to travel are essential.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to health plan members in their residences.
• Perform a medical history review and physical evaluation during visits.
• Coordinate and bridge gaps to care based on health plan guidance.
• Use iPad and clinical workflow to document assessments and findings.
• Travel locally to member homes and conduct virtual visits as needed.
