Job summary

Signify Health, Inc. is seeking a Part‑Time Physician to provide in‑home health evaluations at the member’s residence. You’ll engage individuals, review medical histories and medications, and perform a focused exam using an iPad within a clinical workflow.

The role allows for local travel within licensed states and may include virtual visits. MD/DO licenses are required with multi-state licenses preferred; approximately 29.5 hours per week at $125/hour.

Qualifications

• Active, unrestricted medical license(s) in the applicable state(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes.
• Review medication and medical history; perform physical evaluation.
• Coordinate with health plans to connect members with the right care.
• Travel locally and within licensed states; conduct virtual visits as needed.
