Job summary

Signify Health seeks Part‑time physicians to perform in‑home health evaluations for health plan members. You will connect one‑on‑one with patients in their homes, reviewing medical history, medications, and performing a physical evaluation using a streamlined workflow on an iPad.

Active MD/DO licenses are required with multi‑state options preferred. You may travel locally and within licensed states, and some visits can be conducted virtually as needed to support care access and coordination.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multistate licenses are preferred for broader coverage.
• MDs and DOs eligible to apply.

Responsibilities

• Conduct in‑home health evaluations, ~45 minutes per visit.
• Engage with patients and review medications and medical history.
• Perform physical evaluations and coordinate with health plans for tests as ordered.
• Travel locally and within licensed states; conduct virtual visits as needed.
