Job summary

Signify Health is seeking a Part-time Physician to perform in-home health evaluations across communities. You’ll connect one-on-one with members, review medical histories, perform physical evaluations, and provide diagnostic support as ordered by health plans.

You’ll use an iPad and a streamlined clinical workflow, without prescribing meds or altering current regimens. This role requires MD/DO with active licenses, the ability to travel locally within licensed states, and occasional virtual

• Active, unrestricted license in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations and review medical history.
• Perform physical evaluations and discuss findings with patients.
• Coordinate diagnostics and preventive services as ordered.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Use Signify Health workflows via iPad during visits.
