Qualifikationen

• Active, unrestricted medical license in one or more states
• Experience providing in-home patient evaluations is preferred
• Strong communication and bedside manner to support patients at home

Aufgaben

• Perform in-home health evaluations (~45 minutes) including medication and medical history review
• Conduct physical evaluations and, when ordered by health plan, additional tests
• Work with Signify Health clinicians to connect members with appropriate care
• Be willing to travel locally and within licensed states; may conduct virtual visits as needed
