You will visit patients at home, review medication history, perform physical evaluations, and coordinate with health plans to connect members with the right care. Travel locally within licensed states is required, with virtual visits as needed.

Qualificações

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
