Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to perform in-home health evaluations, engaging with members in their own homes and bridging gaps in care.

You will conduct 45-minute visits, review medications and history, perform physical evaluations, and coordinate with health plans. Requires MD/DO license, multi-state licenses preferred; travel locally within licensed states; part-time schedule and benefits included.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses preferred.

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
