Job summary

Signify Health, part of CVS Health, seeks a Part-time Physician to perform In-Home Health Evaluations with patients in their homes. The role involves 45‑minute visits, reviewing medications and medical history, and performing a physical exam. The clinician will use an iPad workflow and may conduct virtual visits as needed.

Travel locally across licensed states and within the national network is required. Prior to visits, clinicians join a trusted care team to provide personalized health

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for members.
• Review medication and medical history.
• Perform physical evaluation and document findings.
• Coordinate with health plan for additional tests when ordered.
• Travel locally within licensed states as needed.
