Job summary

Signify Health, part of CVS Health®, is seeking a Part‑time Physician to provide In‑Home Health Evaluations. You will visit members at their homes, review medical histories, perform physical evaluations, and coordinate with health plans to bridge gaps in care.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
• Able to cover primary locations: Marquette (Counties: Alger, Baraga, Marquette)

Responsibilities

• Provide in-home health evaluations for health plans members.
• Engage with patients and answer health questions during 45‑minute visits.
• Travel locally within licensed states and conduct virtual visits as needed.
