Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to deliver in-home services. This part-time/per diem role focuses on restoring strength, balance, and independence for clients in their homes.

The ideal candidate holds an MA PT license, a DPT/MPT, CPR certification, and has 1+ year of experience (new grads welcome). A reliable vehicle and Driver’s license are required. Mileage reimbursement is provided.

Qualifications

• Must hold an active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
