Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist for part-time / per diem in-home care. You’ll perform in-home PT evaluations, develop personalized care plans, and deliver skilled interventions to improve strength, balance, mobility, and safety.

Active MA PT license, DPT or MPT, CPR certification required; 1+ year preferred. A reliable vehicle and valid driver’s license are necessary.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
