Job summary

Signify Health is recruiting a Part Time Physician to perform in-home health evaluations, engaging with members in the comfort of their own homes and helping bridge gaps in care.

The role covers about 29.5 hours weekly with a pay range of $105 per hour, travel within licensed states, and the use of an iPad with a clinical workflow to deliver Diagnostics & Preventive Services. Active, unrestricted MD or DO license is required; multi-state licenses preferred.

Qualifications

• Active, unrestricted MD or DO license in the applicable states.
• Multi-state licenses preferred.
• In-home evaluation experience is a plus.

Responsibilities

• Perform in-home health evaluations and health assessments.
• Travel locally to member homes and within licensed states.
• Use Signify Health workflow and an iPad for documentation.
• Provide Diagnostics & Preventive Services as indicated.
• Coordinate referrals to the member's health plan.
