Job summary

The RN will perform comprehensive assessments, develop and revise care plans, administer prescribed medications, and provide caregiver support. Travel to patient homes is required.

Qualifications

• Graduate of an accredited school of nursing and current RN license.
• CPR certification required.
• Experience in home care and hospice care preferred.

Responsibilities

• Completes comprehensive patient and family assessments to determine hospice needs.
• Provides nursing care using the nursing process and plan of care.
• Develops and revises the care plan with patient and family input.
• Explains health instructions and coordinates with physicians and hospice team.
• Coordinates with caregivers to support activities of daily living and comfort measures.
