Job summary

Alc-Health-Care-Services is seeking a Registered Nurse for a PRN hospice role in Illinois. Work is on the road serving patients in their homes and communities, part-time with flexible scheduling. Rates are paid per visit: Admissions $135, Deaths $100, Follow up Visit $75, Recertification $100.

The nurse will assess patient needs, develop and adjust care plans, administer physician-prescribed treatments, counsel families, and coordinate with interdisciplin groups to provide compassionate care.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in Illinois and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.

Responsibilities

• Completes an initial, comprehensive patient assessment in the home setting.
• Develops and updates the plan of care based on nursing diagnosis.
• Administers medications and treatments per physician orders.
• Counsels patient and family on care needs and self-care.
• Coordinates with hospice interdisciplinary team and attends meetings.
