Qualifications

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments.
• Plans and implements patient care using nursing process.
• Develops and revises the plan of care with patient and family input.
• Monitors patient status and communicates changes to physician.
• Provides medications and treatments per physician’s order.
• Counsels patient and family on nursing and related needs.
• Supports activities of daily living and promotes self-sufficiency.
