Job summary

ALC Health Care Services is seeking a part-time Registered Nurse to plan, organize and direct hospice care in home settings throughout the River Grove area. The RN will use nursing process to assess, plan, implement and evaluate patient care in collaboration with families and the hospice team.

The role requires state licensure, CPR certification, and hospice experience is preferred. Excellent communication and physical ability to assist with activities of daily living are essential attributes

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree with hospice care experience preferred.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family.
• Provides professional nursing care using nursing process.
• Assesses and evaluates patient status and initiates care plan.
• Regularly re-evaluates patient and family needs and revises the plan of care.
• Collaborates with physician and hospice interdisciplinary team.
• Delivers preventive and rehabilitative nursing procedures and administers medications.
• Counsels patient and family and provides health instructions.
• Assists patient with activities of daily living and supports self-sufficiency.
