Job summary

Harmony Home Health and Hospice, LLC. seeks a compassionate Registered Nurse to plan, organize, and direct hospice care in patients' homes or communities.

The RN will lead in-depth assessments, develop and implement care plans, and coordinate with the interdisciplinary team to ensure high-quality end-of-life care. You will administer medications, provide patient and family education, and support comfort-focused goals while maintaining compliance with licensure and safety standards in a community

Qualifications

• Graduate of an accredited school of nursing.
• Current state licensure, CPR certification and valid driver’s license must be provided.
• Ability to supervise hospice aides and collaborate with the IDT.

Responsibilities

• Performs initial and ongoing comprehensive assessments of patient needs to determine hospice requirements.
• Assesses admission eligibility and level of care for hospice services.
• Evaluates caregiver ability to meet patient needs and coordinates care planning.
• Provides plan of care, revises as patient status changes, and documents progress.
• Administers medications and treatments as prescribed and educates patient/family on care.
