Job summary

Alc-Health-Care-Services is seeking a PRN Hospice Registered Nurse to plan, organize and direct hospice care in home settings. The role emphasizes community health education and patient/family support, working with an interdisciplinary team.

The RN will assess needs, develop and revise care plans, administer medications, and provide caregiver counseling. Strong nursing skills, licensure, and driving capability are required.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Must be able to supervise hospice aides.
• Strong observation, communication and problem-solving skills.
• Able to lift and assist patients; valid driver's license with insured vehicle.

Responsibilities

• Completes initial and ongoing assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family; provide health care instructions.
• Assist with activities of daily living and support self-sufficiency.
• Communicate with physician regarding patient needs and orders.
• Attend interdisciplinary group meetings.
