Job summary

Alc-Health-Care-Services seeks an experienced hospice RN to plan, organize and direct home-based hospice care, drawing on community resources to meet patient and family needs in their homes and communities.

The role involves comprehensive patient assessment, care planning, medication administration, patient/family counseling, and collaboration with physicians and the hospice team to ensure comfort and quality of life on a full-time basis while traveling to patient homes.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree with one year hospice care experience preferred.
• Excellent observation, verbal and written communication skills.
• Ability to lift and transfer patients; valid driver's license required.

Responsibilities

• Complete initial and ongoing patient and family assessments for hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plan of care with patient and family.
• Regularly re-evaluate patient and family needs.
• Participate in revising the plan of care as patient status changes.
• Initiate and revise the plan of care as needed.
• Use health data to determine nursing diagnosis.
• Administer medications and treatments as prescribed.
• Counsel patient and family in meeting nursing needs.
• Provide health care instructions according to assessment and plan.
• Assist with activities of daily living and promote self-sufficiency.
• Communicate with physician regarding patient’s condition and orders.
• Collaborate with family/caregiver and hospice team to address emotional needs.
• Attend interdisciplinary group meetings.
