Job summary

At Home Quality Care is seeking a Registered Nurse to join our hospice team on a PRN, part-time basis. The role covers care in patients' homes as traveling nurse, delivering comprehensive nursing services and education to patients and families across the community.

The RN will perform assessments, develop and adjust care plans, administer treatments, and collaborate with the hospice team to ensure comfort and quality of life for patients.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Must be a licensed driver with insured vehicle.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Assesses and writes plan of care based on patient status.
• Regularly re-evaluates patient and family needs and revises the plan as necessary.
• Initiates the plan of care and makes revisions as patient status changes.
• Develops a care plan based on nursing diagnosis and goals.
• Administers medications and treatments per physician’s plan of care.
• Counsels patient and family to meet nursing needs and education.
• Provides health care instructions per assessment and plan.
• Assists with activities of daily living and promotes comfort and independence.
• Communicates with physician regarding patient needs and status.
• Works with family/caregiver and hospice team to address emotional needs.
• Attends interdisciplinary group meetings.
