Job summary

At Home Quality Care is seeking a part-time registered nurse to plan, coordinate, and direct hospice services in patients’ homes. The RN will perform comprehensive assessments, develop and update care plans, administer prescribed meds, and educate patients and families on care and comfort options.

Travel to patient homes is required. The role emphasizes clinical nursing skills, strong communication, and collaboration with interdisciplinary teams to meet patient needs in community settings.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting, and 1–2 years in home care.
• Current licensure in State and CPR certification.
• Bachelor’s degree with hospice care experience preferred.

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Assess and evaluate patient status by initiating the plan of care.
• Regularly re-evaluate patient and family needs and revise the plan of care as necessary.
• Initiate and revise the plan of care as patient status changes.
• Develop a care plan based on nursing diagnosis and involve patient and family.
