Job summary

Ennoble Care seeks a part-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Bartlesville and Claremore area, Oklahoma. The APP will work under physician supervision and participate in a pay-per-visit model.

Responsibilities include conducting exams, developing treatment plans, and coordinating care with families, caregivers, and the clinical team.

Qualifications

• Board Certified with a valid OK NP or PA certification.
• 1-2 years of primary care experience with geriatric patients.
• Ability to work independently and communicate effectively.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Bartlesville, OK.
• Assess health by performing exams, obtaining histories, and planning care.
• Communicate with office staff and clinical operations across platforms.
• Deliver patient-centered care to a defined panel.
• Develop impressions, diagnoses, and treatment plans.
• Foster trust with patients, families, and caregivers during visits.
• Order diagnostic testing as needed.
• Prescribe medications and medically necessary interventions.
