Ennoble Care seeks an Advanced Provider (NP or PA) to deliver primary and palliative care to homebound patients in the Muskogee, Oklahoma area on a part-time basis. Work under physician supervision, caring for patients with chronic conditions across a range of programs including remote monitoring and chronic care management.

Seeking Board Certified providers with Oklahoma NP/PA certification, strong clinical skills, and 1–2 years of primary care experience with geriatric patients.

Qualifikationen

• Board Certified with a valid NP or PA certification in Oklahoma.
• Strong clinical skills to assess, diagnose, and treat.
• 1–2 years of primary care experience with geriatric patients.
• Hospice experience preferred but not required.

Aufgaben

• Provide high quality primary and palliative care in Muskogee, OK.
• Perform physical exams and develop care plans.
• Communicate with office staff across internal platforms.
• Order diagnostics and prescribe medications as needed.
• Foster trust with patients, families, and caregivers.
