Job summary

Ennoble Care is seeking a full-time Advanced Practitioner (Nurse Practitioner or Physician Assistant) to provide primary care and home-based palliative services in the Oklahoma City area. You will work under a physician and care for patients with chronic diseases, supporting the QoL and access to care for homebound individuals.

The ideal candidate is board certified with Oklahoma licensure, has strong clinical skills, and 1–2 years of primary care experience with geriatric patients.

Qualifications

• Board certified NP or PA with Oklahoma licensure
• Strong clinical skills to assess and diagnose in primary and palliative care
• 1–2 years of primary care experience with geriatric patients

Responsibilities

• Provide high quality primary and palliative care to homebound patients in the Oklahoma City region
• Perform physical examinations, update medical histories, and develop care plans
• Communicate effectively with office staff and clinical operations across platforms
• Deliver patient-centered care to a panel of patients
• Develop impressions, diagnoses, and treatment plans
• Consult with patients, families, and caregivers during visits, and order necessary tests
