Qualifications

• Active DEA license/prescriptive authority.
• CPR/BLS certification and ongoing professional development.

Responsibilities

• Assess patients' clinical problems and provide care in outpatient and home settings.
• Perform annual wellness exams for senior patients.
• Coordinate continuity of care and manage patient care plans with the care team.
• Travel within the Salem, OR area up to a 30-mile radius to provide home visits.
• Complete charts within 72 hours and maintain HIPAA compliance.
