Job summary

Greater Good Health seeks a Physician Assistant to join a high‑impact outpatient and home‑visit care team in Salem, OR. You will manage senior patients in-clinic and in home settings, coordinate care with a dedicated care team, and focus on preventing hospitalizations while optimizing chronic conditions and patient well‑being.

You will travel up to 30 miles to patients’ homes, perform wellness exams, document thoroughly, and collaborate with clinicians to ensure holistic, patient‑centered plans

Qualifications

• 2+ years of clinical experience as a PA in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care
• Current DEA licensure / Prescriptive Authority post-hire
• Active CPR/BLS Certification
• Home health experience a plus

Responsibilities

• Assess patients' clinical problems and provide care and consultation
• Perform annual wellness exams for senior patients
• HCC coding, verification, and risk adjustment
• Coordinate continuity of care and preventive services
• Travel within the Salem, OR area to provide home-based care (5 days a week)
