Greater Good Health in Medford, OR is seeking a Part Time Physician Assistant to manage high-risk patients in an outpatient clinic. You will provide in-home clinical support, optimizing care and preventing acute admissions.

The ideal candidate is compassionate, collaborative, and works effectively in a team. Candidates must possess an OR license and have a background in primary care or gerontology.

Qualificações

• 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care.
• Current DEA licensure / Prescriptive Authority post-hire.
• Active CPR/BLS Certification.

Responsabilidades

• Assess patients' clinical problems and needs, providing care and consultation.
• Coordinate continuity of care and manage patient treatment plans.
• Execute discharge plans and ensure patient confidentiality.
