Job summary

Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to deliver care for high-risk geriatric patients in assisted living and nursing home settings in Marion and Polk Counties, OR. This part-time role emphasizes geriatrics, chronic disease management, and palliative care within a value-based model.

The position offers a 20-hour workweek with no on-call, no weekends, and no holidays, plus a $30,000 sign-on bonus or $40,000 loan repayment and $2,500 relocation

Qualifications

• NP: MSN or DNP degree with AANP or ANCC certification.
• PA: Graduate of an accredited Physician Assistant program with NCCPA certification.
• Active state license (or ability to obtain).
• Active DEA license or ability to obtain.
• Ability to obtain a collaborative practice agreement if required.
• Reliable transportation.
• Ability to perform physical assessments in various care settings.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute illnesses and chronic disease exacerbations.
• Coordinate testing, diagnostics, and treatment interventions.
• Ensure accurate documentation and ICD-10 coding.
• Collaborate with interdisciplinary care teams.
• Conduct advanced illness and advance care planning discussions.
• Educate patients and caregivers.
• Coordinate care transitions and implement individualized care plans.
• Function independently while supporting quality patient outcomes.
