• 126 hours of CHW Michigan-specific training; MI Bridges enrollment training.
• CAC curriculum completion; bilingual in English/Spanish desirable.

Responsabilidades

• Coordinate care with care managers to support transitions of care.
• Identify social determinant needs and connect to community resources.
• Provide patient advocacy and coordinate referrals to community agencies.
• Maintain ongoing follow-up with patients and document progress.
• Communicate clearly with patients, families and the care team.
