Munson Healthcare is seeking a Community Health Worker to partner with patients, families, and care teams to improve access to care and address social drivers of health. You will coordinate resources, educate communities, and reinforce care plans while avoiding clinical responsibilities.

Qualifications

• Must have high school diploma or GED.
• Experience in health care, public health, or related community services.
• Completion of or willingness to complete a Michigan-recognized CHW training program.
• MiCHWA CHW certification or eligibility is highly preferred.

Responsabilités

• Serve as a frontline public health worker partnering with patients, families and community organizations to improve access to care.
• Provide health system navigation, outreach, education, and care plan reinforcement.
