Job summary

Access Community Health Network in Illinois is hiring a Care Coordinator who will provide care coordination, social support, and outreach to help patients navigate health care transitions and empower them through education and coaching. The role leads intake, manages care plans, uses information systems to track patients by disease and risk, and conducts face-to-face visits.

Bilingual Spanish is a plus, RN or social work licensure preferred.

Qualifications

• Role requires a professional in care coordination with clinical background.
• Minimum two years in a clinical setting.
• Ability to navigate health care transitions and educate patients.

Responsibilities

• Lead intake process and care plan development and maintenance.
• Proactively contact, educate, and track patients by disease, risk, and self-management.
• Lead Patient-Centered Care Planning including assessment, plan, implementation, and evaluation.
• Conduct face-to-face visits according to policy.
• Provide resources to support patient/family self-management to meet goals.
• Assist patients and providers across care continuum in transitions and follow-up.
