Western Healthcare Alliance is seeking a Care Navigator to support our care coordination program. The role involves outreach, data collection, and reporting of health and social information to reduce barriers to care.

You will work within an interdisciplinary team to improve quality outcomes and coordinate care across the health continuum. The position emphasizes multicultural sensitivity, education on community resources, and adherence to safety protocols in community settings, with a focus on

Qualifications

• Two years of progressive related experience with diverse populations.
• Bachelor’s degree or related certification preferred (MA, CNA, LPN).
• Knowledge of health education, motivational strategies and an empathetic approach when working with underserved populations.
• Knowledge of healthcare business and local area preferred.
• Ability to work with underserved communities empathetically.

Responsibilities

• Support individualized care plans addressing physical, behavioral, and social needs.
• Collaborate with clinic staff to provide education and recruitment techniques.
• Coordinate referrals to medical, behavioral health, social, environmental, and community resources.
• Communicate with service delivery partners to ensure efficient care.
• Follow up with patients discharged from hospitals to recruit into care management services.
• Monitor patient progress toward wellness and promote preventive care.
