Job summary

Qualifications

• Experience in a physician health care office or similar setting is preferred.
• Clear oral expression and written communication; able to use EMR systems.

Responsibilities

• Connects clients with healthcare, social services, and community resources.
• Provide health education and information in a culturally appropriate manner.
• Assist clients with applications for healthcare, food, housing, transportation, and other programs.
• Maintain accurate and confidential client records.
• Advocate for clients and help reduce barriers to care.
