Job summary

Western Healthcare Alliance is seeking a Care Navigator to support the care coordination program, navigating care within assigned populations (Medicare/Medicaid). You will work in an interdisciplinary team and organize, review, and report health and social information by phone outreach with multicultural sensitivity.

Adhere to safety protocols and preventive/disease management programs to promote health outcomes.

Qualifications

• Two years of progressive related experience working with diverse populations.
• Healthcare setting experience preferred.
• Bachelor’s degree in related field and/or related certification (MA, CNA, LPN) preferred.
• Or combination of education and experience that enables performance of the full scope of the position.
• Knowledge of health education, motivational strategies, and an empathetic manner working with the underserved.
• Knowledge of healthcare business preferred.
• Knowledge of local area preferred.

Responsibilities

• Support individualized care plans addressing physical, behavioral, and social needs.
• Educate and recruit with clinic staff.
• Demonstrate Seven Domains of Care Coordination in primary care.
