Qualifications

• Minimum B.A. preferred with 2 years experience in social services or equivalent.
• Excellent computer skills and ability to document clearly and timely.
• Mental and physical health capable of meeting job demands.

Responsibilities

• Provide a range of client-centered services linking clients to health care, psychosocial and other services.
• Develop comprehensive, individualized service plans and coordinate services to implement them.
• Advocate for clients, complete documentation, and prepare programmatic reports as needed.
• Input data into databases accurately and timely; maintain confidentiality and professional standards.
• Coordinate referrals, appointments, and utilize community resources to support clients.
• Collaborate with supervisor and regional coordinators to enhance continuum of care.
