Job summary

Positive Impact Health Centers is seeking a Medical Case Manager to provide comprehensive case management to adults in our clinic, following HRSA/Ryan White guidelines. You will work with an interdisciplinary team to develop individualized service plans, monitor progress, and coordinate referrals for medical and supportive services.

Ideal candidates have a social work or nursing background, experience in medical settings, and Spanish-language skills.

Qualifications

• Bachelor's degree in social work, nursing, or related field; BSW/MSW preferred.
• Experience in medical settings and with adults living with HIV preferred.
• Spanish language skills highly preferred.

Responsibilities

• Learn and stay current on HIV/AIDS and case management.
• Provide case management to identified clinic patients per guidelines.
• Develop ISP and monitor progress for ~70 patients.
• Coordinate referrals for medical and non-medical services.
• Assist with eligibility for ADAP and pharmaceutical programs.
• Document patient information in EMR and reporting systems.
