Job summary

Corewell Health in Grand Rapids, MI, is seeking a licensed social worker to collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs. You will conduct assessments, coordinate discharge planning, and refer to community resources to support transitions of care.

Responsibilities include documenting plans in EMR, providing consultation to the care team, and maintaining up-to-date knowledge of regulations and resources.

Qualifications

• Required Master’s Degree in Social Work
• 2 years of relevant healthcare experience post Master’s Degree preferred
• Care management experience preferred
• Experience in individual/family assessment, crisis intervention, grief & loss counseling, and discharge planning preferred
• MSW required for hire in Michigan

Responsibilities

• Conduct psychosocial assessments with patients and families to identify needs and plan transitions of care.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Document assessment and plan in EMR reflecting patient condition, factors, and psychosocial imperatives.
• Provide consultation and resources to healthcare team.
• Maintain current knowledge of community resources.
• Participate in department, regional, or statewide committees as requested.
• Develop knowledge of federal and state regulations related to the role.
• Provide short-term therapeutic support as appropriate for setting and location.
