Job summary

Harborhospice mi is seeking an experienced social work professional to support palliative care services. The role focuses on psychosocial assessment, family-centered counseling, and coordination with an interdisciplinary team to optimize comfort, dignity, and quality of life for patients and caregivers.

The ideal candidate holds an MSW or related master’s degree, along with LMSW/LMSW eligibility in Michigan, and is prepared to obtain Advance Care Planning certification within a year.

Qualifications

• Master’s degree in social work (CSWE accredited) or related field.
• Current Michigan LMSW or LLMSW license or eligibility.
• Advanced Care Planning Facilitator Certification or willingness to obtain within 1 year.

Responsibilities

• Assess psychosocial status of patients, families and caregivers in home or facility settings.
• Plan and carry out interventions based on the psychosocial assessment.
• Provide strength-based, person-centered and family-centered counseling and psychoeducation.
• Coordinate with the interdisciplinary team to maintain and adjust the care plan for palliative care.
• Facilitate Advance Care Planning discussions and linkage to community resources.
• Document outcomes and report to stakeholders within established timelines.
