Job summary

Corewell Health in Grand Rapids is seeking a part-time Social Worker in Hospice West to collaborate with an interdisciplinary team and support patients and families with complex psychosocial needs. The role includes discharge planning, coordination of services, and connections to community resources.

You will conduct psychosocial assessments, communicate with care sites, document in the EMR, and contribute to safety and regulatory compliance while working a hybrid schedule with field visits

Qualifications

• Master’s degree in social work.
• State of Michigan, Limited Master Social Worker Licensure.
• State of Michigan Driver’s license.

Responsibilities

• Conduct psychosocial assessments with patients and families to identify needs and plan care.
• Coordinate with alternate sites of care and community agencies to address patient needs.
• Document assessment and plan in the EMR, reflecting current condition and transition needs.
• Provide consultation and resources to members of the healthcare team.
• Maintain knowledge of community resources relevant to patient care.
• Participate in department, regional, or statewide projects and committees as requested.
• Develop and maintain knowledge of federal/state regulations related to the role.
• Participate in safety initiatives and complete required safety competencies.
• Perform other duties as assigned.
