Job summary

CorsoCare Hospice seeks a licensed Master of Social Work (LMSW) to join our patient-focused team in Michigan. The role delivers psychosocial assessments, counseling, bereavement services, and community education to patients and families in a hospice setting.

The Social Worker collaborates with the interdisciplinary group, maintains timely documentation, participates in IDG conferences, and supports crisis intervention and care planning.

Qualifications

• Must have MSW degree from a school of social work accredited by the CSWE.
• Ability to work independently and make sound judgments.
• Ability to respond appropriately to crisis outside of hospital settings.
• Adaptability to different social, racial, cultural, and religious contexts.

Responsibilities

• Performs initial psychosocial, emotional, spiritual, and bereavement assessments and assists in care plan development.
• Provides ongoing counseling and crisis intervention as needed.
• Maintains comprehensive documentation in patient records in a timely manner.
• Participates in IDG conferences and Bereavement Team activities.
• Advocates for the patient-family unit and coordinates with community resources.
